The missing W9 form contained sensitive personal information, including the customer’s name, Social Security number, date of birth, address, telephone number, and email address. This loss raised concerns about the potential compromise of this data. The SANE program is taking steps to address the situation and minimize any risks associated with the missing information.
In response to the incident, DPH SANE has been working diligently to investigate the situation and determine what led to the loss of the document. The organization is also reviewing its internal protocols and enhancing employee training to prevent similar incidents in the future. They are focused on improving their security measures to protect sensitive information.
Additionally, the incident has been reported to several authorities, including the Massachusetts Attorney General, the Office of Consumer Affairs and Business Regulation, and the Executive Office of Technology Services and Security.
